Résumé de la thèse
Les pathologies rachidiennes de lenfant sont un motif fréquent de consultation en orthopédie infantile. Parmi elles, la scoliose idiopathique de ladolescent est la plus fréquente. Lévaluation de ces patients se fait via la radiographie standard réalisée dans un environnement contraint. Dans ces conditions, il est assez rare de constater des modifications de la posture rachidienne. Or, la simple observation de ces patients à la marche montre que des modifications de la posture existent. Dans létat actuel des connaissances, lévolution de la posture rachidienne, et en particulier son aspect dynamique, est assez mal connue. Les hypothèses scientifiques de ce travail étaient : (1) La posture rachidienne dynamique change au cours de la croissance et de lacquisition dune marche mature et stable ; (2) Les déformations rachidiennes provoquent une chaîne de compensation globale de la posture touchant les membres inférieurs ; (3) La scoliose idiopathique de ladolescent entraine des modifications du schéma de marche et une modification de la posture rachidienne globale et (4) Le traitement chirurgical de la scoliose idiopathique de ladolescent modifie la posture rachidienne dynamique et permet de retrouver un schéma de marche physiologique.
Lanalyse dune cohorte denfants sains a mis en évidence des modifications de la posture rachidienne dynamique au cours de la croissance, avec une augmentation de la gîte du tronc vers lavant au fur et à mesure de lacquisition dune marche mature. Ces modifications saccompagnent de modifications anatomiques, en particulier au niveau des facettes articulaires cervicales.
Nous avons développé un modèle danalyse du rachis en analyse quantifiée du mouvement (AQM). Ce modèle montrait des corrélations significatives avec les paramètres radiographiques usuels dans une population dadolescents porteurs de scoliose idiopathique. Il est donc possible à partir de lAQM, dutiliser les mêmes descripteurs qu'en radiographie standard. Lanalyse dynamique a montré des modifications du schéma de marche, en particulier dans le plan transversal. Nous avons mis en évidence un décalage de phase entre la rotation des épaules et du bassin chez les patients scoliotiques. Il existait aussi une diminution de la gîte du tronc vers lavant chez les patients scoliotiques. Ces modifications nétaient pas corrélées à lamplitude de la déformation. Par ailleurs, il ny avait pas de différence dans le schéma de marche entre les patients ayant une courbure thoracique droite et ceux ayant une courbure lombaire gauche.
Nous avons analysés, à partir des résultats radiographiques, leffet de la fusion vertébrale par montage hybride sur léquilibre global de patients porteurs dune scoliose idiopathique. Ce type de montage permettait lobtention dune fusion intervertébrale de qualité avec une restauration à la fois de lalignement sagittal et de léquilibre frontal. Lanalyse de ces patients en AQM à 2 ans postopératoire a montré une restauration de certains paramètres grâce à la fusion vertébrale. Notamment, cela permettait de faire disparaître le décalage de phase dans le plan transversal. Cependant, un faible nombre de sujets issus de la cohorte initiale étaient disponibles pour lanalyse à 2 ans de recul, diminuant ainsi la puissance statistique de cette étude.
Au total, ces différents travaux ont permis de mettre en évidence des modifications de la posture et de lanatomie rachidienne accompagnant lacquisition de la bipédie permanente chez lenfant et la maturation de la marche. Cette équilibre rachidien est perturbé par la scoliose idiopathique de ladolescent, à la fois dans le plan sagittal et dans le plan transversal. La réalisation dune fusion intervertébrale postérieure permet de corriger partiellement ces modifications posturales. Des études avec une cohorte plus importante de patients pourrait permettre de confirmer ces résultats.

Spinal conditions are ofently seen in pediatric orthopedics. Among them, adolescent idiopathic scoliosis (AIS) is the most frequent. Usually, the evaluation of these spinal conditions is performed through radiographic examination. Because this analysis is performed in a constrained position, changes in global spinal posture is rarely seen in these patients. However, when seeing these patients walking, changes in spinal posture in dynamic conditions is obvious. In the current knowledge, the evolution of the dynamic spinal posture with growth and in pathologic condition remains unclear. The hypotheses of this work were: (1) dynamic spinal posture changes with growth and with the achievement of a mature gait; (2) spinal deformities lead to a chain of compensation affecting the lower limbs; (3) adolescent idiopathic scoliosis leads to changes in gait pattern and to a modification of global spinal posture and (4) surgical correction of adolescent idiopathic scoliosis spinal deformities changes dynamic spinal posture and tends to give these patients a normal gait pattern back.
The analysis of a cohort of healthy children, aged from 4 to 16 years old has shown changes in gait pattern throughout growth. With achieving mature gait, an anterior tilting of the trunk appears gradually. These changes in global spinal balance are linked to anatomical changes, in particular in the cervical facet joints.
We developed a gait analysis model allowing for the analysis of the trunk. In AIS patients, this model has shown significant correlations with the usual radiographic parameters. It means that it is possible, from gait analysis data, to describe the same parameters that are frequently used in radiographic analysis. Dynamic analysis of AIS cohort highlighted numerous changes in gait, when compared to healthy subjects. In particular, we found that the transverse plane was mainly affected by the spinal deformity, with changes in shoulders and pelvis rotation. Additionally, we found changes in the sagittal plane with a decreased anterior tilt of the trunk in AIS patients. These modifications were not correlated to deformity width. Surprisingly, there was no difference in gait pattern between patients with right thoracic curves and those left lumbar curves.
From radiographic data, we analyzed the impact of surgical correction with hybrid instrumentation (sublaminar bands and pedicular screws) on the overall spinal balance of AIS patients. We have demonstrated that this kind of instrumentation allows for a good intervertebral fusion, with restoration of both sagittal and coronal spinal alignment. At 2-year follow-up, we found a restoration of some gait parameters. Of note, fusion of the main curvature allowed to restore shoulder and pelvis rotation. However, only a few number of patients were available for analysis at this follow-up.
At final, these works have highlighted changes in spinal posture and spinal anatomy with the achievement of a mature gait in children. Dynamic spinal balance is modified by AIS spinal deformity, both in the transversal and sagittal planes. By performing posterior spinal fusion and deformity correction, it is possible to partially restore these gait modifications. However, a longer follow-up with a larger cohort could allow to confirm these findings.
