Résumé de la thèse
Le purpura thrombotique thrombocytopénique (PTT) est une microangiopathie thrombotique sévère liée à un déficit en ADAMTS13. Néanmoins, lactivation endothéliale joue également un rôle crucial. Lobjectif de mon travail a été détudier les conséquences de lactivation endothéliale au cours du PTT sur la perméabilité vasculaire, et dinhiber pharmacologiquement la dégranulation endothéliale in vitro et in vivo dans un modèle murin.
Nous avons montré que des plasmas de patients atteints de PTT (vs des plasmas de patients contrôles) induisent un décollement des cellules endothéliales HUVEC in vitro. Ces modifications jonctionnelles impliquent la phosphorylation Ca2+-dépendante et linternalisation de la VE-Cadhérine et cet effet est reproduit par la fraction IgG des patients. De plus, létude de la perméabilité vasculaire chez la souris a montré que les plasmas de patients PTT induisaient une majoration de la perméabilité qui pouvait être corrigée un inhibiteur du flux calcique intracellulaire, lITF1697.
Nous avons testé lITF1697 comme inhibiteur de la dégranulation sur un modèle murin de PTT qui consistait en linjections de fortes doses dIL-1β chez des souris KO ADAMTS13. Malgré un faible effet sur le flux calcique intracellulaire in vitro, lITF1697 na pas permis dinhiber la dégranulation endothéliale et na pas montré deffet protecteur sur les plaquettes dans ce modèle.
Au cours de ma thèse, jai également travaillé sur le COVID-19. Nous avons montré dans une série de 551 patients que léosinopénie profonde était un facteur prédictif des formes sévères de COVID-19. De plus, nous avons montré que lutilisation dIL-1Ra recombinant pouvait apporter un bénéfice chez les patients les plus sévères, témoignant de limportance des IL-1 au cours des états hyperinflammatoires et de limmunothrombose.

Thrombotic thrombocytopenic purpura (TTP) is a severe thrombotic microangiopathy associated with ADAMTS13 deficiency. However, endothelial activation also seems to play a crucial role. The aim of my work is to investigate the consequences of endothelial activation during TTP on endothelial junctions and cell detachment, and to pharmacologically inhibit endothelial degranulation in vitro and in vivo in a mouse model.
We showed that plasmas from TTP patients (compared with plasmas from control patients) induce endothelial cell detachment in vitro, involving Ca2+-dependent phosphorylation and internalization of VE-cadherin, and that this effect was reproduced by the patient IgG fraction. In addition, examination of vascular permeability in mice showed that plasmas from TTP patients induced an increase in permeability that could be normalized by an inhibitor of intracellular calcium flux, ITF1697.
We tested ITF1697 as a degranulation inhibitor in a mouse model of TTP in which ADAMTS13 KO mice were administered high doses of IL-1β. Despite a discrete in vitro effect on intracellular calcium flux, ITF1697 failed to inhibit endothelial degranulation and showed no protective effect on platelets in this model.
During my PhD research, we also looked at COVID-19. In a series of 551 patients, we showed that marked eosinopenia is a predictive factor for severe forms of COVID-19. In addition, we have shown that the use of recombinant IL-1Ra may be beneficial in the most severe patients, highlighting the importance of IL-1 in hyperinflammatory states and immunothrombosis.
