Résumé de la thèse
Le pronostic de la chirurgie dans l'épilepsie temporo-mésiale dépend de la résection complète de la zone épileptogène (ZE), uniquement possible lorsquelle sorganise de manière unilatérale, et quil existe une compensation possible sur le plan fonctionnel des structures controlatérales. Dans cette thèse, l'objectif était d'étudier si les potentiels évoqués liés à lévénement (ERPs) et l'analyse spectrale du signal hippocampique pouvaient contribuer à l'analyse fonctionnelle des structures temporales mésiales chez les patients épileptiques. Deux principaux générateurs d'ERPs au sein des structures temporo-mésiales ont été décrits : l'hippocampe pour la MTL-P300, P600 et LNC, et le cortex rhinal pour la AMTL-N400. L'absence d'ERPs, l'amplitude et la modulation (l'effet de répétition des ERPs) de ceux-ci, ainsi que la quantification de la puissance spectrale hippocampique, ont été décrits comme des outils précis pour déterminer le caractère épileptogène et la fonctionnalité des régions mésiotemporales.
Dans cette thèse, nous avons réalisé deux revues systématiques et une revue de la littérature. En outre, deux études originales ont été réalisées en analysant les données d'une cohorte rétrospective de 81 patients.
Dans notre première étude originale, nous avons constaté que l'absence de la MTL-P300 était significativement corrélée à la participation de lhippocampe dans la ZE. En utilisant une courbe ROC, il a été révélé qu'une amplitude de la MTL-P300 inférieure à -46 µV avait une spécificité de 93,3% pour détecter l'EZ. En revanche, la MTL-P300 ne s'est pas révélée être un marqueur utile pour analyser limplication de l'hippocampe controlatéral à la ZE. Dans la période préopératoire, une corrélation positive a été observée entre l'amplitude du MTL-P300 de l'hippocampe gauche et les performances au test de mémoire verbale.
Notre deuxième étude originale a mis en évidence une corrélation positive entre l'amplitude de la réponse MTL-P300 de chaque hippocampe et la puissance spectrale de lactivité gamma. La puissance des différentes bandes de fréquences delta, beta, gamma étaient significativement plus faibles dans les hippocampes inclus dans la ZE par rapport à ceux non impliqués. Nos résultats ont montré que les hippocampes épileptogènes sans sclérose hippocampique avaient également une puissance plus faible que les hippocampes normaux, cette différence étant statistiquement significative dans la bande gamma. Nous avons montré que cette réduction était plus marquée dans les hippocampes avec une réponse MTL-P300 absente.
Ensemble, nos études ajoutent des connaissances pertinentes sur l'applicabilité de l'enregistrement MTL-P300 ainsi que de l'estimation de la puissance gamma pour latéraliser la zone épileptogène et étudier la fonctionnalité des structures temporales mésiales.

The prognosis of surgery in mesial Temporal Epilepsy depends on the complete resection of the epileptogenic zone (EZ), and on the functional reserve of the contralateral temporal mesial cortex. In this thesis the aim was to investigate whether event-related potentials (ERPs) and spectral analysis contribute to the functional analysis of the mesial temporal structures in epileptic patients. Two main generators of ERPs within mesial-temporal structures have been described: the hippocampus for the MTL-P300, P600 and LNC, and rhinal cortex for the AMTL-N400. The absence of ERPs, their amplitude and modulation (repetition effect of ERPs), and the quantification of hippocampal spectral power, have been described as accurate tools in determining the epileptogenicity and functionality of the mesial-temporal regions.
In this thesis we have performed two systematic reviews and a literature review. In addition, two original studies were undertaken by analyzing data from a retrospective cohort of 81 patients.
In our first original study, we found that the absence of the MTL-P300 was significantly correlated with hippocampal involvement in the EZ. Using an ROC curve, it was revealed that an amplitude of MTL-P300 below -46 µV had a specificity of 93.3% to detect the EZ. In contrast, the MTL-P300 was not found to be a useful marker for analyzing hippocampal involvement contralateral to the EZ. In the preoperative period, a positive correlation was observed between the amplitude of the MTL-P300 of the left hippocampus and performance on the verbal memory test.
Our second original study found a positive correlation between the amplitude of the MTL-P300 response in each hippocampus and the spectral power of gamma activity. The power of the different frequency bands delta, beta, gamma was significantly lower in the hippocampi included in the EZ compared to those that were not. Our results showed that the epileptogenic hippocampi without hippocampal sclerosis also had lower power than normal hippocampi, being statistically significant in the gamma band. We showed that this reduction is more marked in hippocampi with an absent MTL-P300 response.
Together, our studies add relevant knowledge about the applicability of MTL-P300 recording as well as gamma power estimation to lateralize the epileptogenic zone and to investigate the functionality of mesial temporal structures.
