Résumé de la thèse

Nos travaux antérieurs ont évalué l’injection de fraction vasculaire stromale (FVS) issue du tissu adipeux autologue au niveau des doigts chez les patients atteints de sclérodermie systémique, caractérisée par un processus de fibrose atteignant la peau et les organes profonds ainsi qu’une dysangiogénèse responsable de raréfaction capillaire et d’une vasculopathie ischémique. Ces travaux ont montré que cette thérapie cellulaire améliore le handicap de la main et réduit les manifestations vasculaires ischémiques. D’un point de vue réglementaire, la FVS est un Médicament de Thérapie Innovante (MTI) qui correspond à une nouvelle catégorie de médicaments biologiques . Le développement de ces médicaments exige désormais une application stricte des Bonnes Pratiques de Fabrication de l’industrie pharmaceutique nécessitant une connaissance précise de leurs propriétés biologiques ainsi que l’identification de marqueurs d’efficacité. L’objectif de ce travail est d’une part de caractériser les effets antifibrotiques et angiogéniques de la FVS et d’évaluer l’impact du contexte sclérodermique sur ces propriétés angiogéniques. D’autre part, d’identifier une stratégie de monitoring biologique de cette thérapie applicable au contexte sclérodermique. Le premier volet a été de déterminer si la FVS injectée localement peut limiter les processus de fibrose in vivo. Un modèle d’incontinence urinaire chez le porc induit par une section endoscopique du sphincter strié mettant en jeu un mécanisme physiopathologique exclusivement dépendent d’un processus fibrotique a été sélectionné. L’'injection de FVS réalisée précocement ou tardivement s'accompagne d'une réduction significative de la surface de fibrose au profit des surfaces musculaires. Ces modifications histologiques se traduisent chez les animaux traites par une augmentation de la pression urétrale, en lien avec la reprise de fonction du sphincter. Le deuxième volet a été de rechercher si le contexte autologue de la sclérodermie systémique affecte les propriétés angiogéniques de la FVS. Pour cela une étude de caractérisation biologique et des propriétés angiogéniques de la FVS obtenue chez des patients sclérodermiques en comparaison à la FVS obtenue chez l’individu sain a été réalisée. Elle s’appuie sur la collection biologique de FVS « thérapeutique » de patients sclérodermiques issue de l’essai clinique SCLERADEC II. Cette étude a montré une légère altération de la capacité angiogénique sur des tests de Matrigel Plug in vivo, en particulier sur la fonctionnalité des vaisseaux de gros calibre. L’étude des profils transcriptomique a également révélé peu de différences. Le troisième volet a été d’identifier une stratégie de monitoring biologique utile pour évaluer de manière objective l’impact de nouvelles thérapies sur la vasculopathie et la fibrose associées à la sclérodermie systémique. Dans un protocole de recherche biomédicale portant sur 45 patients atteints de ScS, nous avons observé qu’une élévation des microparticules endothéliales circulantes, des progéniteurs endothéliaux d’origine non hématopoïétiques et de la fractlakine étaient des marqueurs indépendamment associés à la maladie. L’élévation des PEC, corrélée positivement au taux de fractlalkine suggère qu’elle constitue une réponse à l’activation inflammatoire de l’endothélium. De manière intéressante, l’élévation des PEC et de la fractlalkine prédisent de manière indépendante le score de sévérité de la maladie et la gravité de la fibrose pulmonaire. Ce travail permettra de progresser dans le développement d’une thérapie cellulaire innovante pour limiter la vasculopathie ischemique et la fibrose à l’origine du handicap des mains chez les patients sclérodermique. L’identification de la fractalkine et des PEC comme des marqueurs circulants de la sévérité de la vasculopathie permettra de les positionner comme des outils pertinents dans le suivi biologique de ces thérapies innovantes.

Our previous work evaluated the injection of stromal vascular fraction (SVF) from autologous adipose tissue in the fingers of patients with systemic sclerosis, characterized by a process of fibrosis affecting the skin and deep organs, and dysangiogenesis. responsible for capillary rarefaction and ischemic vasculopathy. This work has shown that this cell therapy improves the handicap of the hand and reduces the ischemic vascular manifestations. From a regulatory perspective, the FVS is an Advanced Therapy Medicinal Product that is a new category of biologic drugs. The development of these drugs now requires a strict application of the Good Manufacturing Practices of the pharmaceutical industry requiring a precise knowledge of their biological properties as well as the identification of efficiency markers. The aim of this work is to characterize the antifibrotic and angiogenic effects of SVF and to evaluate the impact of the sclerodermic context on these angiogenic properties. On the other hand, to identify a strategy of biological monitoring of this therapy applicable to the sclerodermic context. The first step was to determine whether locally injected SVF can limit fibrosis processes in vivo. A model of urinary incontinence in pigs induced by an endoscopic section of the striated sphincter involving a pathophysiological mechanism exclusively depends on a fibrotic process was selected. The injection of SVF performed early or late is accompanied by a significant reduction in the area of fibrosis in favor of muscle surfaces. These histological changes are reflected in animals treated by an increase in urethral pressure, in connection with the recovery of function of the sphincter. The second component was to investigate whether the autologous context of systemic scleroderma affects the angiogenic properties of SVF. For this purpose, a study of biological characterization and angiogenic properties of SVF obtained in sclerodermic patients compared to the SVF obtained in the healthy individual was carried out. It is based on the biological collection of "therapeutic" SVF from scleroderma patients included in the clinical trial SCLERADEC II. This study showed a slight alteration of angiogenic capacity on Matrigel Plug tests in vivo, particularly on the functionality of large diameter vessels. The study of transcriptomic profiles also revealed few differences. The third component was to identify a biological monitoring strategy that could be used to objectively evaluate the impact of new therapies on vasculopathy and fibrosis associated with systemic sclerosis. In a biomedical research protocol involving 45 patients with SSc, we observed that elevation of circulating endothelial microparticles, non-hematopoietic endothelial progenitors, and fractlakin were markers independently associated with the disease. The elevation of PECs correlated positively with the rate of fractlalkin suggests that it constitutes a response to inflammatory activation of the endothelium. Interestingly, elevation of EPC and fractlalkine independently predict the severity score of the disease and the severity of pulmonary fibrosis. This work will help to progress in the development of an innovative cell therapy to reduce ischemic vasculopathy and fibrosis causing hand handicap in patients with scleroderma. The identification of fractalkine and PEC as circulating markers of the severity of vasculopathy will position them as relevant tools in the biological monitoring of these innovative therapies.
