Résumé de la thèse

Le cancer du poumon est le deuxième cancer le plus répandu dans le monde avec plus de 2,2 millions de nouveaux cas en 2020 et la première cause de décès par cancer (soit 18,0 % du total des décès par cancer). L'essor de l’immunothérapie et plus précisément des inhibiteurs de checkpoint immunitaire a entraîné des résultats cliniques spectaculaires, malheureusement, les taux de survie n’ont pas été ceux espérés notamment avec des taux de survie à 5 ans de l’ordre de 30 à 40 %. Le cancer bronchique non à petites cellules (CBNPC) représente 80 à 85% des cancers du poumon. Le traitement chirurgical et la radiothérapie sont les traitements de référence, mais les patients atteints d'une maladie avancée y sont récusés. Selon les directives du NICE, la chimiothérapie chez ces patients doit être une association de sels de platine avec un nouvel agent (dit de 3ième génération). Le sel de platine le plus utilisé est le cisplatine associé au pemetrexed, tandis que l'agent de 3ième génération est un Inhibiteur des Checkpoints Immunitaires (ICI). Après des premières tentatives d'utilisation en monothérapie, les associations d’ICI avec d'autres stratégies thérapeutiques telles que la chimiothérapie, les thérapies ciblées, les anti-angiogéniques ou la radiothérapie sont actuellement recommandées. La recherche et la détermination d’un facteur prédictif d’introduction des ICI en association à des traitements de chimiothérapie actuellement utilisés en pratique clinique (cisplatine, pemetrexed et anti-angiogénique) a pour but d’obtenir une efficacité antitumorale maximale. Dans ce projet de recherche nous avons réalisé une étude longitudinale de l’impact de différents traitements de chimiothérapie, en monothérapie ou en association, sur les MDSCs, les lymphocytes B et T et les lymphocytes T regs, sur différents modèles de CBNPC. Ces études nous ont permis de déterminer le schéma optimal d’introduction des ICI : anti-VEGF à J1 Chimiothérapie à J3 et ICI à J14. Les tests d’efficacité de ce protocole, ont été menés en comparaison à d’autres schémas d’administration. Ce protocole a permis d’obtenir les meilleurs taux de réduction de l’ordre de -74% par rapport au groupe contrôle à la fin de l’étude. Ce protocole fait également mieux, en termes de réduction de la masse tumorale, que les autres protocoles testés incluant anti-VEGF, chimiothérapies et anti-PD-1 selon d’autres modalités d’administration. L’optimisation des modalités d’introduction des ICI explique la supériorité antiproliférative obtenue avec le protocole déterminé.

Lung cancer is the second most common cancer worldwide with more than 2.2 million new cases of in 2020 and the leading cause of death by cancer (i.e., 18.0% of the total cancer deaths). The rise of ICI as new anticancer agents yielded spectacular progress in clinical outcomes, including 5 year-survival rates now above 30-40%. NSCLC (Non-small cells lung cancer) accounts for the 80-85% of all lung cancers. Surgical treatment and radiation therapy are the standard of care, but patients with advanced disease are precluded. According to NICE guidelines, chemotherapy of advanced stage should be platinum-based with a new (3rd generation) single-agent. The more used platinum agent is cisplatin associated with pemetrexed, whereas the 3rd generation agent is an immune checkpoint inhibitor (ICI). After first attempts to be used as single agents, ICI are to be combined with other therapeutic strategies such as chemotherapy, targeted therapy, anti-angiogenics, or radiation therapy. Determination of predictive biomarkers of efficacy with ICI is challenging, as fully understanding the underlying pharmacology of these drugs is complicated. The aim of this project was to monitor immune cells, i.e.,MDSCs, B cells, T cells T regs, in a longitudinal way after exposure to cytotoxics alone or in combination, on different NSCLC models. These studies determined the optimal schedule for introducing ICI: anti-VEGF on D1 Chemotherapy on D3 and ICI on D14. Efficacy experiment, testing this protocol, was performed in comparison to other administration regimens. This protocol shows efficacy with near -74% tumor size compared to the control group at the end of the study. This protocol also shows better efficacy than the other protocols tested including anti-VEGF, chemotherapy and anti-PD-1 according to different administration schedules. This efficacy study confirms that combining anti-VEGF and cytotoxics with ICI, is a best strategy to increase antitumor efficacy, provided that sequencing and scheduling are carefully selected.
