Résumé de la thèse

Le sunitinib et le cabozantinib sont deux traitements de thérapie ciblée ayant eu une grande influence dans la prise en charge du carcinome rénal métastatique. A l’instar de la majorité des thérapies ciblées orales, ces molécules sont caractérisées par une relation dose-exposition-effet, une forte variabilité ainsi que par un profil de toxicité préoccupant. L’objectif de nos travaux était de participer au développement d’outils dédiés à l’individualisation des schémas thérapeutiques des inhibiteurs tyrosine kinase dans le cancer du rein. Le suivi thérapeutique pharmacologique associé à des approches de ciblage posologique basées sur la pharmacocinétique se présentait comme une stratégie de choix dans la personnalisation des soins en oncologie. Nous avons ainsi développé et validé une nouvelle technique bio analytique permettant d’implémenter le suivi thérapeutique pharmacologique du cabozantinib en routine. Notre but étant de déterminer sa fenêtre thérapeutique dans le cancer du rein afin de pouvoir mettre en place ultérieurement une stratégie d’individualisation des posologies. En parallèle nous avons conduit une étude rétrospective visant à étudier la pertinence de l’utilisation d’un algorithme de ciblage posologique dans une pratique de suivi thérapeutique pharmacologique sur la base d’un prélèvement unique et sans notion d’horaire de prélèvement. Après avoir comparé les différentes prédictions du modèle avec les modifications de posologies empiriques réalisées par le prescripteur, nous avons pu observer que le recours à une approche de popPK aurait permis une probable diminution de l’apparition d’effets indésirables précoces chez des patients initialement surexposés à la dose standard. Intégrer une approche de pharmacométrie innovante permettrait ainsi de développer une médecine de précision en offrant des outils d’aide à la prescription aux médecins tout en les éclairant sur les modifications posologiques à apporter afin d’atteindre la cible thérapeutique le plus précocement possible.

Thesis resume

Sunitinib and cabozantinib are both oral targeted therapies that have had a significant impact on the management of metastatic renal cell carcinoma. Like the vast majority of oral targeted therapies, these molecules are characterized by a dose-response relationship, a high variability and a safety profile as concern. Our main objective was to participate in the development of tools dedicated to the individualization of tyrosine kinase inhibitor therapeutic regimens in kidney cancer and the therapeutic drug monitoring associated with dosage targeting approaches based on PK was a strategy of choice in the personalization of oncology treatments. We have developed and validated a new bio-analytical assay to implement the therapeutic drug monitoring of cabozantinib in routine, in order to determine its therapeutic window in kidney cancer and be able to set up later a strategy of individualization of dosages. In parallel, we realized a retrospective study to evaluate the relevance of using a dosage targeting algorithm in a therapeutic drug monitoring follow-up practice based on a single sampling and without any notion of sampling time. After comparing the different based-model recommendations with the empirical dosage modifications made by the prescriber, we observed that the use of a popPK model would have allowed a probable decrease in the occurrence of early-onset adverse events in patients initially overexposed. Integrating an innovative pharmacometric approach would thus make it possible to develop precision medicine by offering prescribing assistance tools to physicians while informing them on the dosage modifications to be made in order to reach the therapeutic target as early as possible.
