Résumé de la thèse
En traumatologie, le concept dostéointégration fut introduit pour la première fois dans les années 1960 comme létablissement dune connexion directe entre los et limplant, capable de résister aux efforts transmis de lun à lautre. La maîtrise des enjeux associés à lostéointégration est dune importance cruciale, dans la mesure où la négligence de lun ou plusieurs de ceux-ci sont responsables de consolidations osseuses post-opératoire incomplètes, délétères pour les patients et générant un surcoût pour les infrastructures de santé pouvant être estimé, en France, à plusieurs centaines de millions deuros.
Mon travail de thèse sinscrit dans cette problématique et sarticule autour de lamélioration du potentiel dostéointégration de vis orthopédiques par une réflexion sur le design et lapplication dun traitement de surface. Son objectif est double, dans la mesure où il vise (1) à développer un modèle mécano-biologique in silico capable de prédire la reformation osseuse autour dun implant avec traitement de surface et (2) à réaliser des expérimentations sur limpact biologique des propriétés surfaces de limplant, aux échelles nano, micro et macro-scopiques pour paramétrer et valider le modèle.
A partir dune revue exhaustive de la littérature scientifique, jai développé, à laide du logiciel COMSOL, un modèle mécano-biologique de reformation osseuse in silico que jai calibré à partir des résultats dune étude in vivo conduite par Abrahamsson et al. (2004). Ce modèle a permis destimer la reformation osseuse péri-implantaire en fonction du design de la vis orthopédique et du traitement de surface.
Ces simulations ont, en particulier, révélé que lostéointégration des vis orthopédiques est plus importante lorsque la chambre de guérison de l'implant est peu profonde et lorsque la concentration en facteurs de croissance adsorbés est importante à la surface des vis, adsorption qui est majorée par le traitement de surface de limplant.
Ce modèle in silico a été corroboré par une étude in vitro qui, au-delà dune caractérisation physico-chimique dun certain nombre de couples matériau/traitement de surface, a permis de caractériser la capacité dadsorption en fibronectine de ces couples, ainsi que de déterminer la réponse des cellules souches mésenchymateuses en termes de morphologie, de prolifération et de différenciation au contact des surfaces. Cette étude, montre notamment une adsorption plus importante de fibronectine ainsi quune prolifération cellulaire réduite sur les surfaces présentant une mouillabilité croissante. Ces résultats ont ainsi pu être intégrés au sein du modèle numérique proposé.
Sur la base des résultats du modèle in silico, une étude in vivo a été réalisée chez la brebis pour valider le modèle. Après une implantation fémorale bilatérale de vis orthopédiques de titane, lostéointégration de celles-ci a été suivie sur trois temps caractéristiques. Les premiers résultats démontrent une bonne tolérance des brebis au protocole chirurgical employé ainsi quune reformation osseuse au sein des chambres de guérison des implants.
Le travail présenté dans cette thèse construit donc les fondations dun modèle prédictif cohérent, qui permet notamment lintégration dun certain nombre daméliorations, sous réserve dune puissance de calcul suffisante, et qui savère être de plus, un outil éthique et dintérêt dans lanalyse multivariée de lostéointégration. Aussi, son application pourrait être bénéfique à loptimisation détudes animales en permettant en amont la mise en place de nouveaux designs dimplants et la réalisation de premières preuves de concept in silico.

In traumatology, the concept of osseointegration was first introduced in the 1960s as the establishment of a direct connection between bone and the implant, capable of withstanding the forces transmitted from one to the other. Understanding the stakes associated with osseointegration is of crucial importance, insofar as the neglect of one or more of these is responsible for incomplete postoperative bone healing, which are deleterious for patients and generating an additional cost for health infrastructure which can be estimated at several hundred million euros in France.
My thesis work falls within this issue and aims at improving the osseointegration potential of orthopedic screws by working on their design and the application of a surface treatment. Its objective is twofold, as it aims (1) to develop a mechanobiological in silico model capable of predicting bone reformation around an implant with surface treatment and (2) to carry out experiments on the impact biological of the surface properties of the implant, at nano, micro and macroscopic scales to parameterize and validate the model.
From an exhaustive review of the scientific literature, I developed using the software COMSOL, a mechanobiological in silico model of bone reformation that I calibrated from the results of an in vivo study conducted by Abrahamsson et al. (2004). This model made it possible to estimate the peri-implant bone reformation according to the design of the orthopedic screw and the surface treatment.
These simulations have in particular revealed that the osseointegration of orthopedic screws is greater when the implant healing chamber is shallow and when the concentration of adsorbed growth factors is high at the surface of the screws, which is increased by the surface treatment of the implant.
This model was corroborated by an in vitro study which, beyond a physicochemical characterization of a certain number of material/surface treatment pairs, aimed at characterizing the fibronectin adsorption capacity of these couples, as well as determining the response of mesenchymal stem cells in terms of morphology, proliferation and differentiation on contact with surfaces. This study shows in particular a greater adsorption of fibronectin as well as a reduced cell proliferation on surfaces with increasing wettability. These results were thus able to be integrated within the suggested numerical model.
Based on the results of the in silico model, an in vivo study was performed in sheep to validate the model. After bilateral femoral implantation of orthopedic titanium screws, osseointegration of these was followed over three characteristic time-periods. Preliminary results showed a good tolerance of the sheep to the surgical protocol as well as bone reformation within the healing chambers of the implants.
The work presented in this thesis therefore builds the foundations of a coherent predictive model, which allows in particular the integration of a certain number of improvements, subject to sufficient computing power, and which turns out to be an ethical tool of interest in the multivariate analysis of osseointegration. Also, its application could be beneficial for the optimization of animal studies by allowing the upstream implementation of new implant designs and the realization of first proofs of concept in silico.
